Intake Forms
To help us get to know each participant and provide the best possible experience, we ask that all prospective participants complete an intake form before enrollment.
Participants who are 18 years of age or older and make their own decisions should complete the Adult Participant Intake Form.
Parents, guardians, or legal representatives should complete the Parent/Guardian Intake Form for participants who are under 18 years of age or for adults who have a legal guardian.
The information collected helps us learn about each participant's interests, strengths, support needs, communication preferences, medical considerations, and personal goals. This allows us to better prepare for their participation and provide meaningful opportunities for learning, growth, social connection, and community involvement.
All information submitted is kept confidential and is used solely for enrollment and program planning purposes.

These enrollment forms are for participants who have been accepted into the program.
The forms collect important information needed to complete enrollment, including emergency contacts, medical information, authorized pick-up individuals, permissions, and other details that help us provide a safe and supportive experience. Please complete all required forms and submit them prior to your participant's start date.
